Am I eligible?
How Can the Free Clinic Help You?
- Are you working (including self-employed) but do not
have health insurance?
- Are you disabled and not eligible for Medicare?
- Are you unemployed?
If any of these situations apply to you or someone you know,
then the Rockbridge Area Free Clinic may be able to help. You must
live in the Rockbridge area (including the cities of Buena Vista
and Lexington). Our eligibility income is roughly 150% of the
Federal Poverty Guidelines.
If you
- Are an employer that hires part-time workers or
full-time workers at minimum wage.
- Have someone cleaning your home, providing
childcare, doing lawn work, delivering firewood or other
related tasks.
the people you hire may be eligible for Free Clinic services.
Please refer them to us.
Eligibility screenings
We conduct screenings for you to become a new patient or to renew
your eligibity at the Free Clinic three times a week. No appointment is
necessary.
- Monday from 10:00 a.m. to noon (New patients or renewals.)
- Wednesday from 1:30 p.m. to 3:00 p.m. (New patients only.)
- Thursday from 1:30 p.m. to 3:00 p.m. (Renewals only.)
We ask you to provide the following documentation to be a
patient at the Free Clinic (the first two allow pharmaceutical companies to
provide medications and each of these three items is required for either
a new patient or for a renewal):
- Current 1040 Federal Income Tax form
- Proof of household income for one month, such as:
- Pay stubs from your current employer
- Food stamp award letter
- A letter of support from whoever is providing you
shelter and food. It must have a monthly dollar amount
of the support provided and it must be signed.
- Social security checks
- Disability checks
- Unemployment checks
- Current bank statement if you have direct deposit
of any of the above three items
- Tax forms if you are self-employed
- Proof of residence in the Rockbridge area, such as:
- Driver's license or Government identification card with address
- Utility bill
- Any other proof that you reside in Rockbridge County (including Buena Vista and Lexington)
Once you are approved you will be issued a Free Clinic
eligibility card. Please keep this card with you and show it any
time you seek services from the Free Clinic.
